Replace a 
Lost Certification Card
 

We wish card replacement was an easy task. 

However, security, accountability and oversight of ASHI cards
 is very important, for the credibility of all courses.
 

All duplicate cards are $5.00 to replace and 
then, mailed to the holder.

We can only replace your card if verified
on one of OUR rosters.

We don't issue cards for classes we don't coordinate.

Once verified, a duplicate card will be issued 
and sent to your address.
